How about doing the day hiking mileage with a pack, and maybe a little extra
weight?
And you sound like a prime candidate for orthotics even the cheap ones, with
a pad above the orthotic for the bone problem.
Another thought. Taking a tylenol or ibuprophen before you leave on longer
hikes. Won't do much for the blisters but can seriously raise your pain
tolerance while you're working this stuff out. Do pay attention to hot spots
as you might not notice them as soon to treat them.
