Glen,

Basically, I use three things in conjunction:  adequate insulation between
myself and the ground (an inflatable pad), a head cover (a wool knit cap;
if it gets really cold, I use the detachable hood from my down jacket) and
a snack (usually a granola bar and hot chocolate) before I turn in. 

One caution: I've rarely encountered below 25 degrees Fahrenheit, and I do
most of my summer backpacking somewhere in the Rockies. Once or twice I've
woken up to a few inches of snow, and temperatures in the mid-teens. So if
you regularly experience 15-degree Fahrenheit morning lows while
backpacking, adjust my experiences to your own.
