Thanks for the link to Merrell's WIDE size shoes.  Does anyone know of a shoe brand that makes extra wide toe boxes with very narrow heels?

Merrell does make wide sizes.  Most stores do not carry the "wide" because
it doubles their inventory cost.  You just have to know where to look.
