My son: no underwear
My husband: switched to soft cotton boxers on his first long distance hike.
Takes several pairs. He hates being dirty and smelly and wearing dirty
clothes.
Me: 2-3 pr.  I'm known for hanging them by baby diaper pins on the back of
pack while I hike to dry them after I rinse them out at night.

Everyone's body and needs are different. See what works for you.  I don't
carry any other "spare" clothes other than underwear and socks.
