Yes, the Canadian Entry Form also needs to be addressed.

Should something go wrong with the entry form, you have till August or 
September to resolve it. If you're a stickler for details, technically, 
you'll need your PCT permit before you start your hike.

The PCT permit and Canadian Entry Form are simple and easy processes. 
Assuming an April or May start date, if you send in your forms a couple 
months before you leave you should be golden.
