Contact a local independent insurance agent if you are having trouble
locating an insurance plan.  They are basically sales agents for
multiple insurance companies and should be able to give you a range
of options.

That being said, I'd get a high deductible plan.  To decide on
how high a deductible to get, compare the premiums relative to the
amount of out-of-pocket expenses you can deal with should you get
injured.  The lower the deductible the higher your monthly
premiums are going to be.

My philosophy is that I want insurance to cover extreme injury
or illness but not to cover basic health care or an occasional
injury or illness.  Of course this assumes that you have a reasonably
healthy lifestyle to start with.
