If you've ever had a DUI, you will not be issued a permit to enter Canada.
A hiker I know had a DUI many years ago and was denied the permit.
Sly's right about the birth certificate / driver's license combination, but
it really depends on the person at the border. In 2002, Noid, Dewey, and I
showed our driver's licenses and breezed right through. In 2003, I was in a
car with Tin Cup and Rusted Root. I had my passport and was fine. The
border patrol person gave Tin Cup and Rusted Root some trouble. Said she
didn't have to let them into the USA because all they had was their driver's
license. Strangely, she didn't even notice that Rusted Root's driver's
license had expired a year and a half ago. They let him into the country,
but when he tried to get on an Amtrack train to go to Oakland, the Amtrack
people wouldn't let him on the train because of the expired license.
