Monday morning, armchair quarterbacking:

Paraphrased from the Oregonian Newpaper: 
The parties on Mt. Hood apparently were climbing one above the other.  One of 
the climbers in the uppermost party slipped, starting a domino effect of 
dragging his/her partner into the party below, dragging them into the party 
below, dragging all three parties into the crevasse.  


In climbing steep ice it is common knowledge to avoid this by climbing 
parallel to and not directly beneath any party above you.  Real life 
circumstances, however, sometimes make this impossible or impractical in 
narrow couloirs.

Penalty: dire consequences.
