Not necessarily.

If they require hospitalization the border patrol is supposed to drop them off at local hospitals without processing them, and never go back to pick them up. From there they just walk away (if they are able.)

That way the county foots the medical bills. If taken into custody, and processed while in the hospital, the INS would get the bill. 

This "twist" on things has actually led to several self-inflicted wounds. One BP officer I met on a Section A hike told me that he has spoken with several Pollos who had been instructed to injure themselves if apprehended.
