That CORSAR card looks interesting, hadn't heard of it before:

So it's just a voluntary donation, not insurance.  And it doesn't sound
like they only rescue card-carrying members so that's good!

Anyway, time and labor and other costs incurred by the volunteer SAR team
are always free as far as I know.

Transport by third parties (helo, ambulance) can be billed or not, it's up
to that third party.  There are helicopter transport
'membership'/'insurance' options you can join/buy for <$100/yr depending on
your location, but, they only cover transport by certain agencies.  If that
agency's helicopter is unavailable, they may send one that will still bill
you the $20k or so for a ride to the hospital.  And, you don't get to
request one agency or another if you do need a ride.
