I've been using Lonely Planet insurance while hiking the PCT, prices seem reasonable, can enroll for a number of weeks or months. I'm an expat living in Japan and my Japanese insurance doesn't cover time in the US. Foreigners ending up in a US hospital without insurance can be shocked by the bill for even minor visits.
