There is probably a water related reason for this, but it ain't giardia. The primary thing that rural people have is less health care, including far fewer dentists. The secondary factor is that farm youth are exposed to a higher level of pesticides than city youth. Even if children do not work in the fields, they are exposed to the chemicals that do show up in their drinking water. The result is that rural youth have a higher incidence of such diseases as Parkinson's disease.
