I'll echo Mango's recommendation to get a professional diagnosis, and
recommend seeing a physical therapist as well for a whole-body
assessment.  It's possible that pain-free dysfunction somewhere else
in your body is changing the way you load your knees and contributing
to the problem.  (In my case, I had an unstable pelvis and tight hip
flexors, which coupled with a bad gait to cause pain under the balls
of my feet that insoles did not fix.)
